Let me see if I got this right. You want to keep the game realistic?
So in other words, you want to remove all magic from the game? Where the logic
in having fire spring out from your hands?
And when we are removing magic, we must remove all magical things as well. No
more scrolls, potions, mystical objects such as portals etc. No more strange
abilities like clairvoyance, customized movement messages or such.

Where's the realism in that? There is no magic. So from now on you can only
choose nomad or religious backgrounds. When you get hurt in combat, it will
take you half a year of game time to heal that arm of yours before you can
fight again. And nuns and monks can only heal in the means of using herbs to
speed up the process. In a realistic way of course.
And when you die, you DIE. Character gone, start over, period.
You get the point, right? Wouldn't be very fun with an all realistic MUD.
