If there are plenty of the said items in the game, they are usually
downgraded.
If there is only a small amount of said items, they don't affect the overall
balance of the game a whole lot, they rarely make it into the hands of players
that are too newbie to even know what the item is. A lot of times there is
some other item in the game that gives the same kind of stats anyway, making
the ungettable eq just a piece of +status.

and then you proceed to whine and cry about the game and how unfair everything
is? Perhaps you could give examples of monsters that were just killed "a few
times" (especially by those 2-man parties of yours) that have suddenly been
massively upgraded. When you find such an example, you'll probably also notice
that its eq's (even the old ones) have been tuned to the proper level.
