Could items be claimed even if there are names already?
a) first claim would clear all names, second would add your name
b) It would require 2 claims. "A,B and C did the heroic deed bla bla bla. Item
is claimed by D."
And please add somekind of information that shows us how many claims we have,
not all players have wiz friends to tell that.

No. The item claimer was coded to handle specific items that for some
reason do not get the item claim information (i.e. they are being
produced by non-standard code). It was not coded to allow for better
transfer of items between players.
Wizards shouldn't tell your claims in the first place - the amount of
claims available are dependant on your level. Again, this system is
meant to handle non-standard eq (which is becoming more rare as wizards
fix their code). If you use it to tag items that you have not
participated in doing yourself, then we won't be here to help you out
